Pain can be Acute or Chronic in Nature.

Acute Pain is the expected symptom of an injury or disease. Chronic Pain occurs when the pain becomes the problem rather than the symptom of a problem.

Pain is a Normal Part of Living!   Pain can be the result of a complicated condition or simply the results of ageing.

Pain Elimination may not always be a realistic goal but, with Pain Management, you can continue to enjoy life.

The Real Goal of Chronic Pain Management: Achieve the Highest Level of Function Possible Using the Least Amount of Treatment or Medication.

First Visit?

The First Visit, We surveyed new patients about what information they would like to have known prior to their first visit. We compiled these items into a special "frequently asked questions" sheet for new patients who have not made their first visit to our office. Follow this link to see those questions: New Patient Info Oct 08.PDF

Forms! No fist visit to a doctor's office is complete without forms to fill out! We know this is not fun but it is incredibly important to have these forms filled out completely at the time of your first visit,

These forms will be mailed to you when you make your first appointment. You can also download and print the forms you will need to complete before your first visit. New Patient Forms.PDF  Please e-mail us with any questions about these forms.

     Time Saving First Visit Tips!

For your first visit, please plan to arrive about 10 minutes early. Much of the paperwork needed for your new patient visit will be sent to your home prior to your appointment.

Please complete complete all of this paperwork at home before your visit, This will save tons of time and make your visit more pleasant.

Please bring all of your insurance cards and diagnostic (MRI, X-Ray, CT, EMG, etc.) reports so we do not have to call to request them from the office you had the test done at. The Doctor must see these reports before he can see you and recommend treatment.

Please bring any pertinent medical records/reports from other physicians who are treating you or, especially if you have been seen by another pain management physician.

Please bring any pain medications that you are currently taking with you so we can see them. Please understand that there are some prescription medications that you can only have written by one doctor so it is important that we see exactly what medicine is being taken and precisely who is writing the prescription.

*** You may also wish to sign up for our On-Line Newsletter. It is issued about once per quarter or therabouts and you can get on the list by clicking on the "Sign up Today!" button on the upper right hand corner of this page>
